replaced equipment is then warranted for 90 days or the balance of the initial warranty period, whichever
is greater. The one year warranty excludes all base unit backup batteries which are warranted for a period
of 90 days only. Equipment which has been subject to abuse, misuse, alteration, unauthorized repair, or
damage as a result of Acts of God beyond the control of Mytrex, Inc., is not covered by the warranty.
Mytrex, inc. shall have the right of final determination of the existence and cause of defect.
Personal Help Buttons
Mytrex, Inc. warrants the RA3TX and RAT3A PHB for a period of one year. Product warranty eligibility
will be determined by the serial number located on the label placed on the back of the PHB. PHBs with
missing labels will not be replaced under the warranty.
Notifications
Your MXD has been registered with the U.S. Federal Communications Commission (FCC), in
accordance with Part 15 and Part 68.
The FCC requires that you connect your MXD to the nationwide telephone network through a modular
telephone jack (USOC RJ11C, RJ11W, RJ14 or RJ31X). This equipment may not be used with coin
telephone lines or party lines. Contact the state public utility commission, public service commission or
corporation commission for more information.
Notification to the Telephone Company:
Upon request of your local telephone company, you are required to provide them with the following
information:
1.
The “Line” to which you will connect your MXD (that is, your phone number); and
2.
The MXD’s FCC Registration Number and Ringer Equivalence Number (REN). Those numbers
are on the bottom of your MXD. The REN is used to determine how many devices may be
connected to a telephone line.
Excessive RENs on a telephone line may result in the devices not ringing in response to an incoming call.
In most, but not all, areas, the sum of RENs should not exceed five (5.0). To be certain of the number of
devices that may be connected to a line, as determined by the total RENs, contact your telephone
company. For products approved after July 23, 2001, the REN for this product is part of the product
identifier that has the format US: AAAEQ##TXXXX. The digits represented by ## are the REN without
a decimal point (E.G., 03 is a REN of 0.3).
Repair information:
If trouble is experienced with this equipment (MXD personal emergency response system), please contact
your provider for warranty or repair information. If the equipment is causing harm to the telephone
network, the telephone company may request that you disconnect the equipment until the problem is
solved.
Caution
: Changes or modifications not expressly approved by the party responsible for compliance
could void the user’s authority to operate the equipment
Rights of the Telephone Company:
If this equipment (MXD personal emergency response system) causes harm to the telephone network, the
telephone company will notify you in advance that temporary discontinuance of service may be required.
But if advance notice isn’t practical, the telephone company will notify the customer as soon as possible.
Also, you will be advised of your right to file a complaint with the FCC if you believe it is necessary.
