Use the questionnaire provided to ensure that all the proper requirements have been meet when 
installing the PaniniPro. This should be performed by an Authorized Dealer or Service Agent. Answer the 
questions provided with a yes or no answer followed by initialing the line. This form must be completed 
and returned to Nemco Food Equipment. 

Failure to do so will result in the PaniniPro not being covered 

under warranty.
