Registration Audit – RA

Details are listed in date order with the oldest transaction at the top.
Up to 99 transactions may be held per patient. Once the maximum number of
transactions is held, the oldest one will be deleted when a new transaction is captured.

key will be available if transaction details extend beyond the first screen.
Multiple amendments performed simultaneously on some screens (ID for example)
will be counted as a single transaction.
If the patient’s NHS number is changed, a new audit trail will be created for the
patient, using the new number. The change of NHS number transaction will cross-
reference to the patient’s previous number, and vice versa. If other changes were made
simultaneously, then these will be recorded on both the old and new screens.
A change of Prescription Exemption type on the PE screen (i.e. where the type of an
existing entry is changed) is not captured for Registration Audit. All PE Historic
entries are classified under one category – ‘PE Screen (Historic)’.
If the reason for Deduction of a patient is changed on the DP Screen, from O/R to an
O/R sub code or vice versa, or from one O/R sub code to another sub code, this
change will not be captured for Registration Audit.
When a patient is deducted from the Health Authority system, the MR status of the
patient is usually changed by the deduction process. This change of MR status is
implicit from the Deduction (which is included within the audit) and is not, therefore,
captured as a separate audit activity. Similarly, a change of MR status for a patient as
a result of an NHSCR transaction will not be captured on RA.
For a deduction generated by an HA transfer, the transaction will inform you of the
fact. It will also display the identity of the transfer set that the record was transferred
in. Examples 1 and 2 below illustrate a patient’s transfer from one HA (id=QCT119)
to another (id=CR4), showing the RA screen displays at both HAs.
