Understanding the DL Screen (Reason – R/C)
NHS No
The patient’s NHS number. A list of patient NHS numbers can be displayed on the SI
(Surname Index) screen.
Deduct Date
The date of deduction, which must be within the current open quarter. If this field is
left blank, the system will set the deduction date to the current date.
MR Req?
When you attempt to deduct a patient as ‘Registration Cancelled’, the system will
prompt you to determine whether or not you wish to request the patient’s Medical
Records from his/her GP.
If the patient’s M/R status (prior to deduction) is ‘FGP’, ‘EFG’, ‘NOR’ or ‘NOT’,
then this field will default to

If the patient’s M/R status (prior to deduction) is any status other than ‘FGP’, ‘EFG’,
‘NOR’ or ‘NOT’, then this field will default to
N

Eff. NHS
The ‘R/C’ deduction type is used to remove duplicate NHS number patient
registrations from HA Core Systems. This field allows you to enter the other NHS
number under which the patient is registered within the Core System. By entering the
‘effective’ NHS number in this field, you will ensure that this number is both recorded
within Deducted Patient Notes (DN screen) and advised within the Amendment
transaction to the NHSCR which an ‘R/C’ deduction automatically creates.
If an effective NHS number is entered, the text “Effective Patient NHS Number:
xxxxx” is added to that patient’s Deducted Notes (viewable from the DN screen). This
text will also be included within the corresponding Amendment transaction to the
NHSCR.
If no effective NHS number is entered, the text “No NHSCR Action Required” is
added to that patient’s Deducted Notes (viewable from the DN screen) and also
included within the corresponding Amendment transaction to the NHSCR.
Service Desk Incident Number
If you enter an effective NHS number, it is also necessary to enter a Service Desk
incident number (SDIN). The SDIN should have been supplied to you when you
reported the problem and should be a number of up to 10 digits.
