Understanding the DP Screen
CODE
Enter the appropriate action code in this field.

If the message “Additional Details exist” appears in inverse text on line 4
of the screen you can also use this field to view additional notes in a pop-
up window. See page 3-14 for details.
Audit Details Exist
This field allows you to view an audit of changes for the patient displayed on screen.
Press your normal key combination (e.g.
Ctrl Shift –
) to display a pop-up window
showing the audit details.
The field label shows whether or not audit details exist for this patient.
The audit box can only be opened by users with full registration access and only for
those patients who have an audit history.
GP Code
The unique code assigned by your HA to the patient’s GP. The system automatically
enters the GP’s name.
Surname / Forename
The patient’s full name. Valid characters are A–Z ’ or a blank space. The
surname/forename must consist of at least two characters, one of which must be
alphabetic.

Prev Name
The patient’s previous name, for example, the maiden name of a married woman.
Valid characters are A–Z ’ or a blank space. The previous name must consist of at
least two characters, one of which must be alphabetic.
House Name
Address Line 1
Enter all elements of the address that occur before the building number, or (if there is
no building number) before the thoroughfare. Valid characters are A–Z 1–9 , – ’. or a
blank space.
Road / Street
Address line 2
This field should contain the building number and thoroughfare name wherever they
are present. If there are two thoroughfare names, they should both go in line 2 if there
is sufficient space. If there is no thoroughfare name, line 1 should contain the
property’s name and this field should be left blank. This situation would be common
in rural areas when addressing farms.
Valid characters that may be used in this field are A–Z 1–9 , – ’. or a blank space.
Abbreviations should only be used if the complete name will not fit. A list of valid
abbreviations can be found in the glossary.
